- Your name, phone number, mailing address and email contact information.
- Send the puzzle name, and the reason you are requesting a replacement.
- Include the stamped Lot # and Date from the bottom of the puzzle box.
- Cut the UPC “bar code” from the puzzle box and include with the letter.
